The Hub at Curtin is partnering with WA Primary Health Alliance (WAPHA)

The Hub brings together Curtin’s unique capabilities in health systems and health services research, health economics, population health research, data linkage, clinical trials, clinical registry development and education. One of the Hubs key strategic partnerships is with the WA Primary Health Alliance. The role of WAPHA is to oversee the strategic commissioning functions of the three Western Australian Primary Health Networks:  Perth North, Perth South and Country WA.  This new role will be working on a project that is within the Curtin and WAPHA partnership.

Your new role

You will play a pivotal leading role, responsible for the development and delivery of the Belmont initiative. The Belmont initiative aims to develop an integrated multi-disciplinary approach to support patients with chronic conditions in place, close to home. It brings together a ‘care neighbourhood’ consisting of GP practices, pharmacies, allied health and the local government with relevant community services.  In this role you will:

  • Manage all phases of the project, deal with emerging issues and undertake problem solving/troubleshooting to keep the project on track.
  • Map and engage local and other stakeholders relevant to the prevention and management of chronic conditions (COPD).
  • Develop an evaluation framework and program logic in consultation with stakeholders. This includes determining agreed achievable, measurable indicators for clinical benchmarking, project monitoring and evaluation purposes.

You will bring to the role

Your excellence in stakeholder engagement across primary health care combined with your experience in quality improvement processes; as well as:

  • Completion of a graduate qualification in a relevant discipline and extensive relevant experience. Relevant fields are quality improvement, primary care and/or project management.
  • Demonstrated high-calibre planning, management and leadership skills as well as outstanding interpersonal skills, including the ability to engage and influence others.
  • Desirable clinical experience, knowledge of COPD and understanding of evaluation frameworks and program logics.
